The Iowa Chronic Care Consortium is an independent, not-for-profit population health demonstration, training and consulting organization. Its focus is to partner with others to bring effective, personalized health improvement and chronic care strategies to individuals where they work and live – reducing the burden of chronic conditions. Validated population strategies in diabetes, heart failure, hypertension and readmission reductions across the country have driven positive validated outcomes with partners.
