Target Audience
The target audience includes Registered Nurses, Licensed Practical Nurses, Registered Dietitians, Diabetes Educators, Certified Medical Assistants, Care Coordinators, Case Managers, Exercise Physiologists, Pharmacists, Social Workers, Respiratory Care Practitioners, Physicians, Nurse Practitioners and other healthcare professionals who work in medical settings that support patients with chronic conditions. Appropriate settings may include primary care and specialty care clinics, cardiac and pulmonary rehabilitation programs, case management services, “Chronic Illness Management” departments, Heart Failure programs, Diabetes Education programs, Home Health/Visiting Nurse Service agencies, Pharmacies, Registered Dietitian consultants, medical fitness professionals, and health coach consultants.
Continuing Education Credit
Iowa Licensed Nurses
The Iowa Chronic Care Consortium is working in cooperation with the University of Iowa College of Nursing, Iowa Board of Nursing Approved Provider #1, to provide continuing education credits for nurses. For this training, the Iowa Chronic Care Consortium has applied to be approved for 41.5 contact hours (4.15 CEU’s).
Registered Dietitians
This program has been approved for 36 hours of continuing education by the Iowa Academy of Nutrition and Dietetics, on behalf of the commission on Dietetic Registration, the credentialing body of the Academy of Nutrition and Dietetics.

Successful Completion
